2024 Summer Registration

Welcome to the Mazomanie Barracudas Electronic Registration platform for our upcoming 2024 swim season! We are pleased to offer electronic registration and online payment via credit card or Venmo or offline payment via check or cash. The process is simple for both new and returning members. You'll get access to your own private account that will enable you easily declare for swim meets, sign-up for jobs, see your children's swim time histories, maintain your contact information, and more.

You must pay before your swimmer will be allowed to participate on the team.

  • If paying by check, please send payment to Mazo Swim Team 5688 County Road F., Mazomanie, WI 53560 **Preferred payment**
  • If paying by credit card, please plan to complete payment online through SwimTopia or at the team suit fitting on Wednesday, April 5th at the Middle School/High School entrance or at a Spring Clinic practice.
  • If paying by Venmo, please use this QR code to access the team Venmo account or search for @MazoCudas 

Registration Fee Structure:
$115 Per Swimmer
$60 Concession Fee Per Family - $20 for each home meet per family (No food donations requested for regular swim season)

Examples of Fee Structure:

Family with 1 swimmer - $115 Swimmer + $60 Concessions = $175
Family with 2 swimmers - $115/Swimmer ($230) + $60 Concessions = $290

Note: if the registration swim fees will place a hardship on your family, please communicate with the treasurer, Aaron Tigert by e-mail at [email protected], to review scholarship opportunities.

Committees

We are looking for parents to participate in committees to take on tasks before and throughout the season. If you are willing and interested, please choose a group and you will be contacted closer to the season. If you are interested in more than one, please indicate that in the "other" section. *

Swimmer Scholarships

Would you be willing to donate any additional money to help cover scholarships for swimmers? *This money will not be reflected in your final amount due. Please add this onto your total when you pay. *

Photo Release

The Mazomanie Barracudas will use photos and images of the swimmers in team promotional materials, on our Facebook page, on this website and in the Star News newspaper.

By continuing with registration, I agree to allow the use of images and photos of my swimmer by the Mazomanie Barracudas.

COVID-19 Waiver

MAZOMANIE BARRACUDA SWIM TEAM COMMUNICABLE DISEASE RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT

In consideration of being allowed to participate in any way in the program, related events, and activities, I the undersigned, appreciate, and agree that:

I am aware there are risks to me of exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus "severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)", which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof.

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE MAZOMANIE BARRACUDA SWIM TEAM, its officers, officials, agents and/or employees, other participants, coaches, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (RELEASEES), from any and all claims, demands, losses, and liability arising out of or related to any ILLNESS, INJURY, DISABILITY OR DEATH I may suffer, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Liability Waiver

By registering my child(ren) with the Mazomanie Barracudas, I agree to participate (or allow my child(ren) and family members to participate) in the Mazomanie Barracudas, and hereby release Mazomanie Barracudas, its directors, officers, agents, coaches, and employees from liability for any injury that might occur to myself (or to my child(ren) and family members) while participating in the Mazomanie Barracudas program, including travel to and from training sessions, swim meets or other scheduled team activities.


I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other family members, or both, while I (or my child(ren) or family members) participate in the Mazomanie Barracudas program.
